At HFH Healthcare we meet the growing need for complex care by providing specialist nurse-led services to adults and children in their own the homes, in and around London. Working in partnership with the NHS continuing healthcare teams, our enhanced level of specialist clinical support enables individual clients to remain at home even when the level of health care required is highly complex. Our services are provided through locality and specialty focused multi-disciplinary Hub Teams.
Our individually-designed care packages are highly specialised, nurse-led and strongly outcome based. Working closely with our patients and their families, we fully identify their specific needs and the most effective ways to meet them.
All our care is delivered in clients’ homes by skilled and experienced Healthcare Assistants. They undertake an inhouse training programme and are assessed against inhouse competencies.
Effective co-ordination of care staff, resources and logistics support is essential to the effective delivery of field and head office services.
ROLE PURPOSE
The Nurse Trainer is responsible for supporting and developing the nursing and carer workforce through clinical practice and promotion/implementation of relevant education and development pathways.
Reporting to the Director of Nursing and working in close partnership with the Regional Lead Nurse and Nurse Case Managers, the Nurse Trainer will promote and educational culture of evidence-based nursing practice.
JOB SUMMARY
- Provide face to face clinical training for care staff in line with company’s clinical competency framework
- Deliver patient specific training sessions in line with business need
- Lead on the development of training presentations, lesson plans and competency documents in line with NICE guidelines and National Standards
- Participate in regular audit programme of training evaluation to ensure effectiveness of training
- Maintain clinical skills lab including stock and equipment auditing.
- Maintain a robust clinical training schedule in line with business need.
- Lead in the delivery, implementation, monitoring and evaluation of a robust mandatory training programme
- Maintain accurate training records including attendance sheet and updating company database
- Participate in clinical On Call rota.
Leadership & key working relationships
- Report to the Director of Nursing and in a peer group with Regional Lead Nurse colleagues
- Work closely with the Nurse Case Managers to ensure high standard of training focusing on quality and patient safety
- Identify carers who require further development and assess their suitability to be put forward to safely provide care to vulnerable adults and children
- Work with Director of Nursing, Regional Lead Nurse, Nurse case Manager, Regional Care Manager, Care Coordinator,
Service delivery, quality and governance
- Ensure the concept and benefits of establishing appropriately skilled and matched core care teams around clients is understood by all members of the regional team, clients and commissioners and ensure this approach underpins service delivery
- Ensure all clinical policies and procedures are regularly reviewed and documented.
- Ensuring all clinical SOPs are implemented and followed
Professional accountability & Business Responsibilities
- Maintain strictest confidentiality and security of information at all times
- Act in accordance with the CQC, NMC and all regulatory bodies
- Maintain own continued professional development through training, experience, and reflection in accordance with the NMC Code of Conduct 2015
- Be aware of their own individual responsibilities as regards to customer quality, safety and care experience.
- Act as a positive role model to promote team working, respect and excellence whilst conducting themselves in professional and courteous manner to all staff, patients and members of the multi-professional team.
- Establish and maintain effective relationships with line management, teams, patients, families, and outside personnel to achieve the business vision and values and promoting business and team success.
- Ensure that all requirements outlined in procedures and policies are personally adhered to.
Clinical, Compliance and Training
- Maintain clinical updates and evidence of PREP (Post Registration Educational Portfolio)
- Attend yearly appraisal with DoN
- Undertake annual appraisal and supervisions of all Nurse Case Managers within region
- Ensure that all customer Safeguarding issues are raised in accordance with HfH Healthcare policy.
- Work with local safeguarding teams including strategy meetings to ensure patient safety within community
- Work alongside clinical audit and risk teams.
- Monitor, review and update clients in a timely manner as discussed with DoN
- Ensure that carer clinical supervisions are completed for all care staff at regular intervals.
- Provide clinical advice and support where appropriate to all office and care staff with regards to clients
- Train carers and nurses and assess competencies of carers and nurses working within caseloads
- Attend all mandatory training and updates
- Participation in the clinical back-up for the on -call service.
