Work on an agreed programme in GP practices within Devon working closely with GPs, other practice staff and members of the primary healthcare team in supporting the development and application of high quality, safe, cost effective, rational, and evidence-based prescribing Work and support on agreed projects and initiatives across Devon as required Support practices and designated localities / areas to implement the medicines optimisation agenda Promote awareness of medicines management and optimisation issues Undertake agreed pieces of work in GP practices which could include completing audits, medication changes and communicating directly with patients as required Monitor performance of designated practices in relation to the agreed medicines optimisation agenda The role supports the business in driving transformation as well as value for money in planning, commissioning, and service The post holder will undertake reporting and analysis of information to support delivery The job description and person specification are an outline of the tasks, responsibilities and outcomes required of the role.
