Job description
An exciting opportunity has arisen for a Continuity of Care worker to support Humankind’s service delivery within Reconnected to Health in HMP Durham reception prison. The main duties of the role will include working as part of a team to provide collaborative continuity of care for service users transitioning into, between and out of Prison Establishments. You will also be required to work in collaboration with Recovery Co-ordinators to ensure effective access to appropriate recovery interventions in accordance with agreed recovery plans and individual need/priorities. You will manage calls which may include screening enquiries, prescribing queries, and booking appointments via an electronic diary.
