Due to the success of the current team manager in securing
career progression a vacancy has arisen for a person to join the Countywide CHC
Team as the CHC Team Manager.
We are looking for a team manager to work with us in the
local authority countywide team working closely with our internal colleagues
and those in the CHC teams for the NHS Integrated Care Boards (ICBs) of Humber and Yorkshire (York and North Yorkshire
place) and West Yorkshire (Bradford Place) plus other neighbouring CHC ICB
Teams.
The team commenced in September 2020 and was expanded and
established as a team in 2022. The team are very supportive of each other and
continue to have regular catch ups 3 times weekly in addition to practice
sessions some jointly with the NHS and team meetings.
The role of the team is to determine eligibility for Continuing
Healthcare and predominately to be a member if the multidisciplinary team to
support the completion of the decision support tool and to agree an eligibility
decision.
The team also arrange and support reviews jointly with ICB
colleagues for people who are not eligible for NHS Continuing Healthcare but
who have needs beyond that which local authority can support and have agreed
joint needs and funding to review.
As the representative from the local authority, you will
need in depth knowledge of the care act eligibility to be able to determine the needs
that may be beyond that which social care can provide. You are also expected to
have a strong working knowledge of NHS Continuing Healthcare processes and
Framework.
Our team is countywide, accessing multiple office bases and
continues to work virtually for some DSTs and some Joint Review meetings being completed
using MS Teams. An increasing number are now being held face to face.
On this basis, location can be flexible across the county.
Travel will be required for meetings including for quarterly face to face team
meetings and to support supervision.
As a team manager you will be providing day to day support
and advice to the team, managing their caseloads and working with other
managers, assessors, business support and health colleagues .
Referrals for CHC Discharge to Assess (D2A) Checklists, DSTs and Joint Reviews are received directly from health and other
local authority colleagues and require urgent response to screen and allocate
to meet local and national timescales.
The team has been successful to date meeting previous
service demands which in conjunction with best practice led to the team
becoming established.
The role will include but is not limited to:

Screening /
Triage of new referrals: Checking for Ordinary residence and that assessments
needed are in place. - Allocations of cases: Working with business support to ensure appropriate
allocations for work coming in, working with ICBs to book in priority work to
reduce breaching timescales wherever possible. - Management
of LLA team Tray (Liquid Logic Adults System): To work with business support for referrals coming into the
tray to ensure that outstanding cases are proactively follow-up for outcomes
from the ICBs. - Management
of CHC email tray: To respond to any queries within the CHC email tray that are
beyond what business support can respond to. - Support:
To the team including arranging catch-ups and practice sessions. Support to
locality Team Managers regarding CHC pathways. - Development: Raising and supporting to unblock any challenges in the pathway
and to improve the systems and outcomes. Development of good practice around
CHC within the team and build on guidance to support wider practice. - Dashboard /
Data / Performance Management: Working to ensure that the collated data is
accurate and that CHC work is being completed in a timely way. Weekly update
for resilience on the service. - Safeguarding: The team support with the information gathering stage for
cases open to the team when the safeguarding is raised and as a Team Manager
you will need to respond screen and allocate to coordinate this including
liaison with other teams and transfer of the safeguarding where further work is
required. - Partnership
Working: Maintaining good working relationships with our partners externally
and colleagues internally.
There is no direct budget management responsibilities at
this time within this post although you will be expected to ensure cost
effective service provision and work with budget managers & team managers
to manage the service and provide the support needed. For cases being held for
joint reviews where there is a new service provision scheme of delegation will
need to be followed.
