Permanent contract | £45,395 to £49,379 per annum (Scale L) | 37 hours per week | Community Services Western Locality - Discharge Team
An exciting opportunity has arisen for a Team Manager to join Norfolk County Council Western locality within our established Adult Hospital Discharge Team based in Kings Lynn Norfolk.
In this role you would be leading and managing a committed and motivated team of social care staff working within a fast pace acute and community hospital discharge team. The team comprises of 3 elements, our integrated Home First Hub, the follow-on team, and the community hospital team. The team consists of practice consultants, social workers, and assistant practitioners.
You will be working as part of an integrated service alongside our community health care and acute health colleagues to ensure we provide the highest quality of hospital discharge for West Norfolk residents. You would be responsible for leading and motivating the team to deliver a high quality and effective service. You will implement policies and procedures in line with local and statutory guidance. You will have the opportunity to lead on the future development of hospital discharges.
The ideal candidate will have strong experience of supervising and performance managing a staff team and advising on the assessment of needs and the management of risk including in relation to Safeguarding enquiries, as well as excellent interpersonal skills whilst working in a fast-paced environment. As a qualified and registered social worker or occupational therapist you will be responsible managing a budget and ensuring the delivery of key performance measures.
You would be joining a supportive and forwarding thinking locality management team and would have the opportunity to lead in specific portfolio areas and contribute to the development of local projects including the Coastal Frailty Project. Kings Lynn has direct rail links into London Kings Cross and the area offers reasonable priced houses to buy and rent.
