JOB TITLE: Team Leader - Learning Disabilities
LOCATION: Tonbridge
CONTRACT TYPE: Temp
WEEKLY HOURS: 36 Hours
WORK PATTERN: Mon-Fri (9am-5pm)
JOB SUMMARY:
To lead by example coaching, motivating and assisting the team to deliver support services of exceptional quality which meet the needs of individual customers and stakeholders whilst exceeding performance targets. To deputise in the absence of the Project Manager to ensure continuous delivery in all aspects of the service.
RESPONSIBILTIES:
- Support the Project Manager to ensure the terms of commissioned contracts and service specifications are met. Take day to day responsibility for appropriate support hours being delivered to customers in both accommodation and community services. And that these are recorded and evidenced in line with commissioning requirements.
- Ensure team rotas are effectively managed and adequate cover is provided for all accommodation schemes and Clarion led community services. Manage and monitor an efficient, timely and fair referrals process. (Where appropriate).
- Establish and maintain a wide range of effective, professional relationships with local relevant agencies and stakeholders. Support the team to carry out appropriate customer needs and risk assessments and develop customer centred support plans
- Support the team to maintain and regularly review support plans, risk assessments and risk management plans. Promote a positive, 'can-do' customer-focused culture in the team. Encourage, enable and support the facilitation of customer involvement as well as offering opportunities for customers to help shape the services we provide
EXPERIENCE:
- Practical experience of delivering front line services to a high standard and providing support services to vulnerable people. Experience of working with vulnerable people and an excellent understanding of the problems they face.
- Ability to be self-motivating and to prioritise workload along with good organisational skills and demonstrable ability to stay calm under pressure and in emergency situations, An understanding of professional boundaries
- Knowledge of Safeguarding Adults and Children I.T. literate and competent in the use of Microsoft Outlook, Microsoft Word, iplanit, Orchard housing, dream24 and general file management. Ability to quickly learn new systems.
- An understanding of Health & Safety and its application Knowledge & experience of support planning, risk assessments and risk management.
Also, will be required to go through an enhanced DBS check.
