Responsibilities:
- Provide person-centered care and support to individuals in their own homes, care homes, or hospitals promoting their independence and well-being.
- Assist with daily activities, including personal care, medication administration, meal preparation, and household tasks.
- Foster positive relationships with service users, promoting dignity, respect, and inclusion at all times.
- Ensure accurate and timely documentation of care activities, including daily records, incident reports, and support plans.
- Collaborate with other healthcare professionals, families, and external agencies to ensure the best possible outcomes for service users.
- Respond to emergencies and crisis situations with empathy, composure, and appropriate interventions.
- Continuously monitor and evaluate the effectiveness of care plans, making adjustments as necessary to meet changing needs.
- Maintain confidentiality and adhere to all policies, procedures, and regulatory requirements.
Requirements:
- Previous experience as a Support Worker or similar role is desirable but not essential. We welcome applications from individuals with a genuine passion for care and a willingness to learn.
- A caring and empathetic nature with a commitment to providing person-centered support.
- Excellent communication and interpersonal skills, enabling effective interaction with service users, colleagues, and other stakeholders.
- Ability to work independently and as part of a team, demonstrating reliability and flexibility in fulfilling shifts.
- Basic knowledge of health and safety practices in a care setting.
- A valid DBS check (or willingness to undergo one) and satisfactory references.
