DUTIES and RESPONSIBILITIES
The duties and responsibilities for a Support Worker which are expected to be fulfilled by him or her are as follows:
· To operate in accordance with Heath Care Services Ltd’s Health and Safety Policy. This includes Health and Safety checks, risks assessments, and cleaning duties to ensure that the home’s premises are kept clean and tidy at all times.
· To encourage and motivate young adults to improve and maintain their independence within their limitations.
· To undertake Keyworker and/or Co-Keyworker roles.
· To report to the Keyworker on any significant matter regarding a young adult.
· To establish a good working relationship with the service users.
· To assist in creating and maintaining service user’s support network, including family, relatives, friends, voluntary and other organisations and professionals.
· To strive to improve the quality of life for the young people and to ensure that their identified needs are being met.
· To arrange health and other non-medical appointments as required.
· To administer and manage medication as per Heath Care Services Ltd’s Medication Policy.
· To prepare and cook meals for the young people in the home as required and as per Heath Care Services Ltd’s Food Safety policy.
· To have appropriate regard to service users’ cultural needs.
· To support young people in the community as required and as per Heath Care Services Ltd’s Lone Working policy.
· To carry out cleaning and other household chores which are not very dissimilar to the usual chores one would expect to do in their own home and as per Heath Care Services Ltd’s Health and Safety and COSHH policy.
· To actively take part in the weekly planning for service user’s.
· To make/answer telephone calls to/from professionals, to write/read letters/emails and to distribute received mail as per etiquette set out by Heath Care Services Ltd.
· To complete all administrative records accurately and as per the organization’s procedure.
· To observe, record and collate information for the preparation and implementation of Support Plans, Behavior Management Plans, Skills Learning plans, Person Centered Plans and Risk Assessments.
· To participate in the updating and reviewing of service users’ plans and risk assessments.
· To contributing towards the reviewing of the homes risks assessments and procedures.
· To collate information for updates and reports for the service users so that keyworkers’ tasks of preparing these reports are facilitated.
· To plan each shift after the handover and record the tasks allocated to each person on duty on the handover sheet.
· To carry out basic personal care tasks that are not the responsibility of a Nurse but which could normally be expected to be undertaken by a member of the family when this is stated in the Support Plan and when you have been trained to do so.
· To attend 100% of all meetings and take minutes when you are the appointed minute-taker. To organise/attend/chair house meetings as appointed by the manager.
· To attend 100% of all trainings provided to you.
· To carry out additional duties not listed above as part of your role in order to achieve the organisation’s aims and objectives.

SKILLS and SPECIFICATION
The skills and specifications that are required in a person to be a Support Worker at Heath Care Services Ltd are as follows:
- The person needs to have the enthusiasm for working with young adults properly and must be able to manage behaviors that challenge services.
- The person needs to have good administration skills and be able to work under pressure.
- It will be definitely an advantage for the person having the knowledge of residential and/or work experience with younger adults/youth work.
- It will be also required for the person that he or she should have passed through DBS checks.
EDUCATION and QUALIFICATION
- The qualifications that is needed for the job is a Health and Social Care Working with Young People Diploma Level/ NVQ Level 3 or other equivalent qualification. Any staff member who does not hold this qualification needs to be enrolled on this course within 6 weeks of their employment.
OPERATIONAL RELATIONSHIP
The Support Worker will be accountable to the Registered Manager through the Team Leader/Deputy Manager. The Support Worker will report to the Team Leader/Deputy Manager.
