We are looking to appoint a Support Worker on a full time basis

You will be working to support service users at KCW Early Intervention in Psychosis Service, alongside our specialist multi-disciplinary team.

We work with young people aged 14-35 who have experienced a first episode of psychosis.

The successful applicant may have contact with patients or service users. As an NHS Trust we strongly encourage and support vaccination as this remains the best way to protect yourself, your family, your colleagues and of course patients and service users when working on our healthcare settings.

You will support our mental health practitioners and other team members in the monitoring of risk, the assessment and review of need where appropriate and development, implementation and evaluation of care plans.

You will work directly with clients and families to support engagement with care plans, both at the team base and in the community

You should have demonstrable experience of working in a healthcare setting and experience of community based working would be advantageous.

You will need excellent communication skills in order to be a client advocate.

PRINCIPAL DUTIES

  • Where appropriate, to motivate service users to actively participate and engage effectively with the agreed care plan, following relevant policies of working with people with mental health.
  • To build a rapport with individual service users and their carers and to respond in a supportive way to service users and their carers, observing the service guidance on professional boundaries, and seeking support from colleagues as necessary.
  • To successfully communicate with service users face to face or by telephone who present with challenging behaviour or who may disengage with services.
  • To provide support to service users so that they can continue to participate in social or other occupational activities or re-establish a meaningful social
  • Working within well defined procedures and practices, to implement directly and in association with others, individual daily living activities working in a variety of environments including independent and shared accommodations and residential settings, towards independent or maintaining community living and according to the individual’s care .
  • To facilitate the empowerment of service users and carers in the planning and delivery of their services and provide, as necessary, practical support in developing and managing dignity and

  • To co facilitate carer support programmes and group work sessions aimed at staying well and promoting
  • To promote the service users' use of community facilities as much as possible within existing resources and promote positive and informed choice making.
  • To support the provision of holistic person centred, flexible support packages which meet the person’s assessed needs with the focus on social inclusion and recovery
