At the British Red Cross, we put the people that need us at the heart of what we do. As Support Worker, you will provide the support and care which can make a vital difference to the quality of people’s lives, allowing them to continue living at home in comfort.
You will provide reactive, focused and flexible support to adults following a hospital stay to enable early discharge when clinical needs have been met. You will assist the Independent Living Service Coordinator by undertaking duties required to meet the service users’ needs in order to re-enable service users to return to their own home and prevent hospital re-admission.

The Independent Living (IL) & Crisis Response (CR) service (IL/CR)supports people in the space between hospital and home and responds to the needs of people in crisis following an emergency; maintaining an extensive network of external relationships across health, social care and emergency services.
You will have the following skills/experience
- Ability to assess service users’ needs whilst completing and following a support plan
- Able to plan own workload
- IT literate with Microsoft 365
- Knowledge of services provided by the NHS and Social Care
- Ability to work as part of a team and on own initiative
- Good Interpersonal and verbal communication
- Working knowledge of health and safety practices
- Understanding of the Risk Assessment process
- Ability to communicate with health and social care professionals
