This Support Coordinator role is valuable in making changes in people’s lives, supporting them and their carers. You’ll be part of a supportive team, working closely with health and voluntary sector partners. You are going to have a big impact on what we do, so will be listened to and trusted to take ownership of your work.

What it’s like working in our team…
You will be part of a close, hard working, fast paced, multi-disciplinary team, working together to help people remain as independent as possible and ensure they have the support that they need, when they need it most. The work can be challenging but is very rewarding. You will be supported by your team and by the Council to develop your skills and progress in your career.
You will be supported to carry out the following duties, responsibilities and training…
- Complete Trusted Assessor Training, so you can assess for and prescribe basic equipment to promote independence.
- To make phone calls to the public and professionals – recording information, referring to the correct person or team. Signposting, offering advice and information.
- To visit people in their home environment and carry out assessments or signpost to where support can be provided.
- To apply a strength-based approach, using the conversations model to connect people to their community/social network and problem solve together.
- Work with people, including advocates and carers, to develop person-centred plans, designed to meet needs in ways that maximises independence, choice and control, and are legal and affordable.

What we’re looking for…
We are looking for someone who is keen to learn and develop new skills, and who wants put these skills to use, to help people be as independent as they can be, keeping the individual at the centre of the plans.
