Like most places, the average age of our population is rising as more people are living longer, so we are working closely with residents to help them maintain their independence and further improve the cost effectiveness of our service to meet the ongoing rise in demand.

Kingston is home to people from diverse and interesting cultures and backgrounds, which helps to create the rich sense of community shared across the borough. In particular, Kingston has a large Sri Lankan Tamil community and is home to more people from Korea than any other part of the UK.

About the Role

The successful Support Coordinator will work within both the Access and Hospital team consisting of the Manager, Deputy Manager ,Social workers, Support coordinators and Senior Social workers.

The Access Team is the front door to Adult Social Care including Safeguarding triage.

The Hospital Team facilitates discharges from Kingston Hospital and all out of borough hospitals

You duties will include:

  • Liaising with the hospital and professionals to facilitate timely discharges.
  • Screening and actioning referrals via our contact centre.
  • Arranging support packages and placements
  • Completing Care Act Assessments to determine ongoing care needs.
  • Providing information, advice and signposting
  • Work in partnership with health and other professionals
About Us

The Hospital Team facilitates discharges from Kingston Hospital and all Out of borough hospitals. The team works alongside multiple professionals to complete Discharge to Assess. We provide a 7 day service. We work alongside the Transfer of Care Hub situated in Kingston Hospital and have a daily presence with a member from the team. The Transfer of care Hub consists of multiple professionals working together to facilitate swift and suitable discharges destinations

The Access Team is the front door to Adult Social Care.

Our referrals come from our residents and professionals via the contact centre.

We triage all safeguarding concerns to establish whether section 42 is required.

About You
  • Experience/Knowledge of the Care Act 2014, the Mental Capacity Act 2005 and other relevant legislation.
  • Experience of working within an Adult Social Care environment
  • Demonstrable experience of creating effective working relationships with internal and external customers
  • Strong literacy, numeracy, interpersonal and IT skills
  • Willing to work some weekends as part of a rota
  • Hybrid working - which includes working from home , providing a presence in the Transfer of Care hub and Guildhall.
