Staying Well Worker

Based at Hanson Lane Enterprise Centre

22 hours per week

Salary £26,008.46 per annum (actual pro rata £15,258.30)

Fixed term until March 2024

Halifax Opportunities Trust is seeking to appoint a Staying Well Worker to join the ‘Central Halifax Staying Well project. The successful candidate will help to effectively engage adults experiencing social isolation and/or loneliness in the Central Halifax Staying Well Hub area.

Key responsibilities:

· To help adults’ access activities within their community which meet their agreed health and wellbeing needs.

· To support the Care Closer to home model of care by supporting people to maintain independence, self-care and avoid unnecessary hospital admissions

· Effectively generate and manage caseloads of referrals from a variety of sources

You should apply for this role if you have

· Experience of working with adults from a wide range of backgrounds

· Excellent communication skills

· A genuine interest in working with vulnerable and isolated people

· Level 2 Literacy

· Experience of working within a community setting and/or with people from disadvantaged groups

· A full UK driving licence and use of car for work.
