We are Trafford Local Care Organisation - an exciting, new public sector organisation that is bringing together NHS community health services and social care services in the city. We’re driven by the aim of improving the health of local people in Manchester by working across organisational boundaries.

We are currently recruiting to permanent roles for our Community Nursing sites in the South area of Trafford based in Altrincham focussing on both transitional and on-going care of patients within a community setting:

The post holder will assess, plan, evaluate and deliver all relevant aspects of patient care in a caring, compassionate and sensitive manner. Work closely with the multi-disciplinary team and contribute to the clinical governance process. Act as a mentor/preceptor to junior staff, with specific responsibility for pre-registration students.

The post holder will take delegated responsibility for the management of the clinical team and the maintenance of high standards of quality patient care on a shift basis.

The hours of the service are from 8am until 10pm over 7 days and a variety of shift patterns will be available.
