Are you passionate about making a difference for children and young people with disabilities and working with a multi-agency team of professionals?

An exciting opportunity has arisen for a part-time permanent Specialist Health Worker to support the Children's Disability Health Team in Ceredigion. We are a small team support across Ceredigion to make a difference to the children and families lives.

The main focus of this role is to enable children and young people to reach their full potential, have equal opportunities, choices and rights in their own community.

The successful applicant must have the ability to assess, plan, co-ordinate and review children with disabilities, and their families informing their health care and support.

The role incorporates being a source of health expertise in relation to disabled children including child development, managing challenging behaviours, health promotion and prevention, identifying early intervention opportunities.

You will need to have experience of working autonomously and managing a caseload, with the ability to support professionals, families, carers and communities with health information and advice in relation to children and young people with disabilities.

The post holder is also responsible for participating in the development and delivery of the health service provided by the Children's Disability Health Team.

The successful applicant will be expected: -

To address the health agenda for each child or young person.

To manage a caseload of disabled children having responsibility to identify and assess the health, social and educational needs of the child in partnership with other family members and other professionals, and to prepare and monitor individual care and support plans.

To participate in the development and planning of services for children with disabilities.

To offer specialist health advice and guidance to the wider multidisciplinary team.

To undertake specialist support with children and their families for example bowel and bladder and behaviour management.

To promote independence and self-care skills by supporting and teaching children and their parents/carers in collaboration with multi-agency colleagues to address day to day issues such as managing their health condition, behaviours that challenge, toileting and sleep programmes.

To prepare health reports/updates for Child Protection and Looked After children meetings.

To contribute and participate in the assessments and reviews of services that have been provided to meet assessed needs.

To work collaboratively with colleagues in the wider Children's Community Nursing Service to address the health agenda.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;

Numerous locations providing mental health and learning disabilities services;

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
