A great opportunity has arisen for an enthusiastic Dietitian with some experience, to join the Harrow Community team based at Alexander Avenue in Harrow.
The post involves contributing to our extensive Harrow GP clinic service where you will work as an active member of the primary health care team. This will allow you to continue to develop your skills in the care of patients with Diabetes, Obesity, feeding difficulties, malnutrition, etc.
The team specialises, in Home Enteral Nutrition, with extensive input across Harrow.
Group teaching sessions for patients, especially in diabetes has continued, even during the current pandemic and this is a key focus for the team. Harrow has a diverse population and a wide selection of nutrition related conditions are prevalent within the Community.
In addition training a wide range of community health professionals such as practice nurses, community nurses and Nursing home staff. Your caseload will include nursing homes patients and those in their own home, alongside the rest of the team, with a focus on Nutrition support.
A car driver is useful, but not essential to the role. However, you will have access to a shared car, for your use.
Community dietitians provide a specialist dietetic service to GP practices, Nursing Homes and patients in their own homes, across the borough of Harrow. They also run regular education sessions to patients groups, such as diabetes, weight management and Coronary Rehabilitation for the Community setting.
The GP outreach clinics provide input to a wide range of patients, this includes: diabetes mellitus, coronary heart disease, weight management, IBS, malnourished, feeding difficulties or poor nutritional status patients. The training of Health professionals, such as Community and District Nurses, alongside health promotion activities targeted at the local Community

The post-holder would be expected:
- To deliver and evaluate the specialist community dietetic service to all patients in Harrow CCG. This involves the Dietetic outreach clinics, working in designated GP
- To deliver and evaluate the specialist dietetic domiciliary service to patients in Harrow CCG
- To provide education and support to the general public in Harrow as part of the health promotion activities carried out by the Community Dietitians.
- To ensure work is in line with departmental, Trust and National Dietetic standards.
- To provide training to General Practice staff and other members of the primary healthcare
- To lead and carry out regular clinical audit into this specialist area.
