We have changed the way that we structure our Longer-Term support teams in Cambridgeshire and have an opportunity for you to join our East Cambs Adult Social Care Community Team.

We are looking for a Social Worker to join on a full time, permanent basis.

The team are responsible for supporting customers who live in the community in the East Cambs geographical area, giving the ability to develop working relationships with customers, their families, and our providers. We are based at the Princess of Wales Hospital and there is plenty of free car parking on site. The team works closely with our health and social care partner organisations and are involved in a number of projects to promote community focused work to transform care provisions in East Cambs. We are proud to be involved in a 3 year project to change the way the Local Authority commissions support to enable people to remain in the community for longer. This includes the development of Individual Service Funds and preventative work with local health colleagues and community based agencies.

What will you be doing?

You'll need to be inquisitive and able to work with others to build working relationships with people and their support networks. You'll need to have excellent observation skills to identify strengths and work with others to promote their independence. Completing Social Care Assessments and Care and Support Planning, completing Safeguarding Investigations, Mental Capacity Act Assessments, Continuing Health Care checklists and attending Decision Support Tool meetings and engaging with Advocacy Services will be part of the role but you do not need to be an expert, full training and support will be provided.

Your curiosity should also mean that you enjoy keeping up to date with current good practice, doing relevant research and building your own knowledge.

There will be an opportunity to broaden your own skills and training by working alongside our health and social care partnership agencies as well as being involved in projects within the East Cambs area, developing your knowledge to promote improved community focus working, and you will support providers and share your knowledge and experience.

Our Community Teams are currently working to a hybrid model, with part-remote and flexible options available in line with our service needs. Once fully trained, our Social Workers will work from the office up to three days each week. This gives our Social Workers a quiet space and plenty of time for reflection whilst still balancing collaborative working and peer support. It has also increased our Social Workers autonomy and has had a positive impact on wellbeing and work-life balance.

About you

Experience or a willingness to work with a variety of partner organisations is vital to working in this team and you'll need to enjoy networking and influencing others to see things differently. You will be able to work autonomously and have the flexibility to manage your own workload in a hybrid way that allows you to work from the office base at least twice a week and from home.

Most of all, you put the individual at the heart of everything and you ensure their choices are being respected and supported.
