We are busy embedding strength-based approaches into everyday practice in our Learning Disability Team and with an eye on innovation, we foster a creative culture that encourages new ideas from our staff and from our residents. This is an exciting opportunity for the right person to make a real contribution to Southampton City Council and to support the achievement of the council outcomes for the city.
This post will involve:
- Completing assessments and reviews of social care needs and plans
- Devising creative care and support plans,
- Supporting carers
- Exploring alternatives to funded social care such as use of care technology and community assets.
- Experience personal budgets and direct payments would be an advantage
- Working in accordance with the Mental Capacity Act 2005 and completing mental capacity assessments
- Experience of safeguarding adults
- Understanding of commissioning arrangements for adult social care support.
