Be part of a team which has a Reablement service which, last year, helped hundreds of people become independent again after a stay in hospital, where Occupational Therapists, domiciliary rehab staff and social workers find innovative ways to support people to live how they like using direct payments, where an incredible partnership with health means integrated teams which deliver services more effectively.

You will be key support for service users in achieving increased independence, working to promote their wellbeing and daily living skills by teaching, assisting in personal care and motivating people in re-learning activities as part of the outcome based Reablement plans designed by Occupational Therapists. You will be making a true difference to people’s lives!

  • To work collaboratively with health and social care community and acute colleagues, including Local Access Point and Discharge to Assess teams.
  • To work with other members of the MDT to ensure effective assessment, management of care pathways and transfers of care / discharges.
