- Salary: £36,298 - £41,496 per annum (pro rata) plus a Local Weighting Allowance of £1,002 per annum & Shift Allowance is also payable.

Department: Reablement & Independence Team - Hours of work: Full time 37 Hours, working on a shift pattern with a requirement for some evening and weekend working

Be part of a team which has a Reablement service which, last year, helped hundreds of people become independent again after a stay in hospital, where Occupational Therapists, domiciliary rehab staff and social workers find innovative ways to support people to live how they like using direct payments, where an incredible partnership with health means integrated teams which deliver services more effectively.
You will be key support for service users in achieving increased independence, working to promote their wellbeing and daily living skills by teaching, assisting in personal care and motivating people in re-learning activities as part of the outcome based Reablement plans designed by Occupational Therapists. You will be making a true difference to people’s lives!

What you’ll be doing:
- Contribute to MDT meetings and facilitate the transfer of short term care support to locality teams where individuals are identified as having longer term care needs.
- To facilitate service users and carers in the identification of appropriate assessments and support plans
- To hold a caseload of a level of complexity and provide case management, guidance support and expert advice to the MDT
- Provide support, advice and guidance to clients and those in their support network which builds on their interests, skills and experience and/or promotes independence.
What you’ll have:
- To work collaboratively with health and social care community and acute colleagues, including Local Access Point and Discharge to Assess teams.
- To work with other members of the MDT to ensure effective assessment, management of care pathways and transfers of care / discharges.
- Demonstrate positive attitudes and behaviours, taking a strength based approach to assessments.
- Excellent time management skills. Including the ability to work to deadlines
- Experience of working with people of diverse cultural and religious communities
- MSC/ Bsc/ Social Work
- Professional Registration (Social Work England)
- Full Driving Licence with access to a car on a daily basis
