About the role

Hospital Adult Social Care Team / Two Full-Time Social Worker Positions

Applicants are required to be both qualified and registered Social Workers, as well as able to travel effectively.

The Hospital Adult Social Care Team is a vibrant and fast paced team, providing social care support to all North Northants residents who need an assisted discharge from hospital.

We take referrals for North Northamptonshire residents from all hospitals, but predominantly from Kettering General Hospital (KGH) Northampton General Hospital (NGH) and the Medical Rehabilitation hospitals run by Northamptonshire Healthcare Foundation Trust (NHFT).

In line with current national guidance, we operate a Discharge To Assess (DTA) process which requires us to carry out timely assessments for those people discharged to DTA beds in care homes in the North of the County and for people discharged to their own homes with DTA packages of care or to the two Specialist Care Centres (SCC’s) in the North of the County.

North Northamptonshire Council is on an exciting innovation journey working towards the Three Conversation approach, a model that re-emphasises focus on people, their strengths and the networks through the development of Community and Inclusion links and hubs. The role of the Hospital Adult Social Care Team is evolving with this approach and has become increasingly more Community orientated, and less hospital based.

In addition to this, North Northants Adult Social Care also offers enhanced rates of pay for those who undertake additional duties on top of their day-day Social Work role.

For those who become Practice Educators, Approved Mental Health Professionals, or Best Interest Assessors (in addition to their Social Work role), and meet the base qualifying criteria, we offer a further £2000 (Pro Rata).

This means there is the potential to earn an extra £4,000 on top of your base salary.

What will you be doing?

You will be working as part of a Team consisting of social workers, senior assessment and enablement officers and assessment and enablement workers who are based at KGH, the 2 SCC’s acute sites and who work from home. Staff provide a range of social care interventions and statutory safeguarding functions to people with enduring and complex needs, as well as their families and carers and with people who may be at risk of harm who are in receipt of or eligible for adult social care interventions.

The remit of the team is to ensure timely, safe and appropriate discharges and to ensure flow through the hospital system, working closely with partners from health and the voluntary sector.

Our work is framed by the requirements of the Care Act 2014 and the need to ensure delayed discharges are kept to a minimum. The team has an ethos of ‘Think Home First’ and re-ablement is pursued whenever possible to promote independence and reduce or delay the need for ongoing support.

The role is currently Monday to Friday but this may change in the future to cover some weekend and BH working, on a rota basis, in line with 7 day working requirements.

About you
  • Applicants need to be a qualified and registered Social Worker. Alternatively to have achieved a degree in Social work within 2 months of the closure date of this advert and to be working towards being registered.
  • Have the passion to promote people’s strengths and the ability to make a difference in a safe working approach to social work practices.
  • Have up to date knowledge of legislation and ideally be able to demonstrable experience of delivering social care outcomes to people in need. Applications from recently qualified or about to be qualify Social Workers will be welcome.
  • Have good communication and IT skills
  • Able to prioritise, using a risk-based approach.
