The Afan MDT Team is seeking to recruit three experienced and newly qualified Social Workers on a full time basis. The posts are newly created to support the expansion of the team within our evolving local authority which is seeking to transform the way that support adults.

The teams work in partnership with adults who have varied needs, their carers and families to identify what matters to them, devise care and support plans to meet their outcomes, whilst maximising independence where possible. The focus of the teams are to build on the assets of individuals within their own communities and working with colleagues to identify this where appropriate.

We work alongside a range of professionals within the teams, including colleagues from the health board, therapies and children’s services. It is an exciting time to join adult social care with friendly, experienced and supportive colleagues who are dedicated to providing a service that puts citizens at centre of our work.

What we can offer you;

An engaging welcome into the team with support and guidance from peers, colleagues and management when you commence you’re your new role, which will continue throughout your career within Social Work at NPT Council.

Meet & Greets with the team are available prior to interview to give you an opportunity to find out more about the services and what it is like working within the team on a daily basis.

There are also opportunities for more experienced social workers, such as training to become an AMHP or Practice Educator, and enrolment on management and leadership training courses. NPT Council invests in their staff, this is an opportunity to join a local authority who will support your development.

Ideally you will have;

Applicants will need to be social work qualified and professionally registered as a social worker with Social Care Wales.

You will need a knowledge and working practice of the Social Services and Wellbeing (Wales) Act 2014, to include outcome focused assessment and a working knowledge of the Mental Health Measures (2010), Continuing Health Care, the Mental Capacity Act 2005 including Deprivation of Liberty Safeguards.
