An exciting opportunity has arisen for a Social Worker to join our Adult Mental Health Social Care Team in North Lancashire (Lancaster and Morecambe)
As part of this team you will complete social care assessments, support plans and reviews designed to support the delivery of a coordinated response to individual's needs. The team works in partnership with all internal and external professionals involved in assessment and service provision, eg. Health, police, housing etc. including Court work when required.
You will need to be someone who can provide professional support to members of the team as well as contribute to the provision of high quality assessments, reviews and support plans for individuals.
There is a full time role working across Monday to Friday 8.45 to 17:00.
