As a Social Worker, you will support young people between the ages of 18 – 25 years old to build a brighter future. It’s a great time to join and help shape this new service as part of a new multidisciplinary team and make a real impact.
You will build individual rapports to support people to engage in their treatment plans and collate information relating to service users and their care plans.
This post holder will be an integral member of the Multi-Disciplinary Team responsible for assessing the social care needs of patients in a Young Adult Eating Disorder Clinic. They will be a principle provider of a quality social work service to address those needs as outlined in patient care plans. This will involve work with young people who have complex mental health and psychological needs.
Your responsibilities will include:
- To meet the standards set out in the General Social Care Councils Code of Practice for Social Care Workers.
- Undertake a comprehensive assessment of the patients’ social care needs.
- To actively participate in the MDT care planning process.
- Contribute towards completion of patients CPA documents providing comprehensive assessment reports when required.
- To agree and adhere to a supervision contract with social work colleagues and Patient Safety Lead as required frequency according to the SWE.
- To comply with the provisions of the Health, Safety and Welfare at Work Act 1974 and other relevant legislation and policies, ensuring the provision and maintenance of a safe environment for all members of the hospital community.
To be Successful as a Social Worker you will need:
- Social Work qualification and eligible to use a protective title.
- Registered with Social Work England.
- Knowledge of social policy and expertise in social work practice
