An exciting opportunity has arisen to appoint an experienced Social Work England registered Social Worker to join the busy Holderness Adult Community Wellbeing Team based in Hornsea, working on a blended approach to include home working. The team covers a wide geographical area and the ability to drive is essential for which a mileage allowance can be claimed.
You will work in accordance with the Care Act 2014 undertaking person centred assessments in the community or hospital setting, assisting in the creation of collaborative care and support plans and monitoring and reviewing support thereafter. Social Workers also participate in the 'duty rota' which can involve dealing with urgent situations. An understanding of policy, operational practice and procedures relating to adults and carers is essential.
You will work within the legislative framework relating to adult social care and will need to be able to demonstrate your ability to work in accordance with the Mental Capacity Act 2005, The Care Act 2014 and a good working knowledge of Deprivation of Liberty Safeguards both in residential settings and community settings.
Excellent communication skills, an ability to manage your time effectively, working under pressure and being flexible in your approach are essential requirements of the role. You will also possess excellent written and IT skills and be expected to input data on the adult information database which is essential in collating data for the performance agenda as well as compiling a complete individual electronic social care record. Performance expectations are set for the team as a whole and on an individual basis in order to meet demand and enhance the customer journey.
All new staff will receive a comprehensive induction, regular supervision, employee development reviews and training opportunities. The team currently operates between Monday and Friday however a move to seven-day working is under consideration.
