Job description

• Working with residents and families to complete Personalised Care and Support Plans -Support and assist other staff members to deliver care as delegated and directed - Completing baseline observations, and supporting the team with appropriate assessments for new residents - Reviewing care plans -Approaches each individual with care, compassion and sensitivity ensuring that these values are reflected in all aspects of the role -To be involved in education for care home staff - To be involved in local projects relevant to improving the health and wellbeing of care home residents within the Apex locality - Liaising with other services - Actively involved in the weekly MDT meeting
