Main Duties: Working in a fast paced review project team, working with client's from the Transition Service, Disability Services and Older People Services. Working with clients, families, carers and providers. Working with multiagencies, partners and commissioners. Carrying out Care Act 2014 Reviews; safeguarding triage; CHC checklists; DSTs; MCAs and BIAs; and challenge panel applications.
