An exciting opportunity has arisen for a Social Work Assistant to be part of a new and dynamic integrated multi-disciplinary team, SCOPE working with individuals with a learning disability and/or autism who have complex care requirements. The team will be supporting individuals from the age of 14 years and upwards at different life stages, from transition from school into adulthood, moving to their own accommodation and beyond into older age.

The team will be made up from disciplines from both Health and Social Care, such as social workers, Outreach Workers, Psychology and aligned to NHS services that include Speech and Language Therapist, Physio Therapy, Dietetics, Psychiatry and Community Learning Disability Nurses. The team will link with local partners such as Education and Children’s services, Housing, other Health Services and 3rd Sector. SCOPE will be leading in developing locally based provision, which will improve the lives of those with complex care requirements as well as their families and carers. The team will have a whole person approach and will embed the principles of positive behavioural support into the delivery of provision. The role of Social Work Assistant in the team will be undertake annual reviews which of clients who are known to the team. Reviews are an essential part of the monitoring of resources which are in place to meet the outcomes of the individuals who are open to the SCOPE team. The role would benefit from an individual who has an understanding and interest in working with individuals with a learning disability and/or autism.

We are looking for an enthusiastic individual who can demonstrate they have experience of working with young people and adults who have a learning disabilities and/or autism. As the Social Work Assistant within the team you will be required to undertake a person centred approach ensuring those individuals and their carers/families are at the centre of your work. You will have an ability to undertake robust reviews which will enable you to update plans to ensure the outcomes of the individuals are being met. As well as have the flexibility and skills to respond to changing needs of individuals. Ideally, you will be able to demonstrate experience of multi-disciplinary working and be committed to working closely with colleagues from other disciplines within the team. You must be able to work effectively within a team and demonstrate a commitment to improving service provision. You will be have good organisational skills and be able to manage a caseload. Excellent verbal and written skills are essential.
