Working 37.5 hours a week, you will join the team at St Mary’s in Warrington. In this role, you will liaise with service users families/significant others as you gather information to assist in carrying out assessments and social circumstances reports to support service users to identify the best outcomes for their health and wellbeing.
From supporting service users health progression to promoting their independence, you will correspond and liaise with external agencies to facilitate and progress arrangements to meet service users set outcomes.
You will be responsible for accompanying service users on community leave and will support them in understanding and managing their personal budgeting and financial arrangement

Your responsibilities will include:
- Maintain regular patient contact on the wards within the hospital.
- Complete Department of Social Security forms with each patient on admission and undertake ongoing welfare benefits work with patients.
- Assist patients in liaison with external agencies regarding financial matters, e.g. payment of bills, etc.
- Liaise with housing agencies regarding patients tenancies, housing benefits and rent enquiries.
- Record all contact with patients and external agencies in clinical and social work notes.
- Liaise with patients families regarding payment of travel expenses and visiting arrangements for families, carers, children and any other significant people.
- Liaison with external agencies regarding social care issues, e.g. Social Services Departments, NHS Trusts and Probation Service, Primary Care Trust.
- To assist Social Workers within the department in collating information relating to patients, care plans.
To be successful in this role, you'll need:
- Good interpersonal and oral communication skills including negotiating mediation and presentation skills.
- The ability to work in a secure setting with patients who present with challenging behaviour and who have a mental disorder.
- Good written skills which can adapt to differing needs.
- Demonstrates effective work planning and time management.
- The ability to practice within the ethos of the Social Work Department’s philosophy.
- Knowledge of welfare benefits and housing related benefits.
- Respect of patient and agency confidentiality
- 1 years’ experience in working with service users in community, social care or health setting.
