POST: Sheffield Pathways Worker
LOCATION: Sheffield GP Surgeries & Abbeydale Road
GRADE: 3, Salary: £25,200 (FTE) £10,800 (Actual)
HOURS: 15 hours per week
Sheffield Pathways supports people in Sheffield and surrounding areas with any crisis they may experience, such as welfare and benefits, substance use, mental health and health & wellness.
Purpose of the Job:
Reducing the attendance in Primary Care Services with social issues.
· Assisting people to manage their current crisis and signposting individuals to engage with other supportive services
· Engaging with the individual presenting in crisis and supporting them by delivering a wide range of interventions, to include:
- Advice and information about local services
- Acting as an advocate for services e.g.; utilities
- Delivering evidence-based interventions
- Completing relevant paperwork and updating monitoring systems
Main Duties
· To be able to provide appropriate, evidence based de-escalation and harm reduction interventions around a number of key issues.
· Supporting people who access the benefits system.
· To adopt an assertive outreach approach to ensure engagement of the service users from the wider community and primary care.
· To support service users to improve general health, including dietary needs, personal hygiene, ensuring basic Maslow hierarchy of needs are met by encouraging uptake and assisting clients to access primary and secondary health care and other appropriate services.
· To support service users to improve their social situation, including assistance with housing, benefits, accessing community services and budgeting.
· To work with users of the service to remove any structural and social barriers to accessing services.
· To support users of the service who have appropriate levels of social capital to access appropriate support.
