Service Manager
A new and interesting opportunity has arisen within the Transformation & Partnership Team, where we are looking to recruit to a Service Manager position on a 12 month fixed term post. The role will offer people an opportunity to work across Adult Social Care departments by managing an operational team whilst also supporting wider Change Programmes with aspects of project related work. Due to the variety of this role it would be an opportunity to build on your existing skills and learn new ones
We are looking for someone who is creative and enthusiastic who would enjoy gaining valuable experience in both operational and transformational work as part of their career development. You will be someone who by nature is an inspirational and motivational leader who is an advocate for better outcomes for people by embedding strength-based and outcome focussed approaches.
You would be expected to work effectively and build positive relationships with stake holders such as client’s, carers, care providers, and health. You will need to be committed to ensuring practitioners work in a person centred and strengths-based way with clients and their families to maintain and maximise their independence. The work will include working with area operational Prevention and Personalisation Teams as well our Contracting Team.
There will be office working as well as working from home. You will be lead a team of Social Care Practitioners and a Senior Practitioner. Part of the role will involve leading and overseeing monitoring reviews/reassessments of people already in receipt of care taking a county wide approach. You will be responsible for gathering and analysing data and work closely with others to ensure we keep on track to meet desired outcomes and any key performance indicators. Part of this work will also involve writing or contributing to reports as well as attending meetings where information may need to be presented. You will play an integral part of embedding any recommended changes from this project across operational teams.
