Job description

Croydon Continuing Healthcare team are seeking a highly motivated and enthusiastic individual to join our team of Placement Officers. The role is to coordinate and lead the process of securing appropriate placements and care support for clients eligible for Continuing Healthcare funding. This will include building effective relationships with relevant professionals and key stakeholders (i.e. patients and their families, nurse assessors, PHB brokers, local authorities, hospital discharge teams, placement and care providers). The successful candidate will have excellent communication skills and be able to work to tight deadlines and under pressure. Equally they should have proven skills in negotiation and have a keen eye for detail.
