Job summary
An exciting opportunity has arisen for a Senior Health Care Assistant to improve the physical health of people with severe mental illness. We are looking for staff keen to raise the profile of physical health care in mental health services.
A small team will provide a clinic based and outreach service using flexible approaches to meet the needs of service users under the care of the community mental health teams. You will deliver evidence-based care for service users either at home, or within a community based clinical environment. You will work independently and collaboratively across teams.
The successful applicant will demonstrate an ability to improve the physical health of people under their care. You may represent your team as a champion and should demonstrate an interest in physical health.
The role includes supporting the nurse in the delivery of comprehensive service user assessment, planning, implementation and evaluation of person-centred care in a collaborative way. The post holder will support the nurse in co-ordinating a service that includes assessment and monitoring of physical health, maintaining service user’s rights, working collaboratively with the Multi-Disciplinary Team and maintaining a safe and therapeutic working environment and accurate records.
