We're on the lookout for a passionate, experienced, and talented individual who can help us create an exceptional environment that nurtures and supports our amazing students.
We are seeking to appoint a skilled practitioner to develop, continually improve and promote our ambitious vision for excellence in supporting students with special educational needs (SEND) within the Directorate of Inclusive Learning.
As a SEN Transition Officer, you will support the Specialist Support Lead with the provision for High Needs students and the Education Health Care Plan process, including Annual Reviews and Initial Assessments across Sunderland College and Hartlepool Sixth Form.
As the identified SEN contact, your role will involve facilitating effective communication and collaboration between schools, partner agencies, and external organisations to support the educational needs of students with special needs.
We believe the key to success is open communication, shared goals, and a commitment to providing the best possible educational experience for students with SEN. Your collaborative efforts with learning support coordinators and curriculum teams will make a positive impact on the students' educational journey.
By fulfilling these responsibilities, you'll be playing a crucial role in fostering a collaborative and supportive environment that ensures the success and well-being of students with special educational needs.
