An exciting opportunity has arisen for an enthusiastic and motivated GP to join a friendly clinical team in Long Melford Practice. We are looking for a 6 session GP to join our team and help us in providing Primary Care to our local community in Suffolk. Main duties of the job In accordance with the practice timetable, as agreed, the post-holder will make themselves available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, patient test results, reports and other paperwork and correspondence in a timely fashion. Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation. Assess the health care needs of patients with undifferentiated and undiagnosed problems. Screen patients for disease risk factors and early signs of illness. In consultation with patients and in line with current practice disease management protocols, developing care plans for health. Contribute to weekly practice meetings About us The Long Melford Practice is based in the historic wool town of Long Melford in Suffolk. We have approximately 9500 patient and use SystmOne clinical system across two sites including Lavenham. The Practice is open to patients between 08:00 am and 18:30 Mondays to Fridays. Our practice is made up of multiple staff consisting of GPs, nurses, reception/care navigators, clinical administrators and dispensary staff who provide services to our patients. We are looking to appoint an enthusiastic salaried GP who will complement our existing team and help us to provide safe and efficient services to our patients. Contribute to weekly practice meetings Please note applications may close before the deadline, so please apply early to avoid disappointment Person Specification Qualifications Essential Fully qualified GP with GMC registration Annual appraisal and revalidation (when appropriate) Experience Essential Good understanding and experience of chronic disease management and primary prevention & screening services Ability to take responsibility for your own career development, learning and performance and are responsible for ensuring that all your mandatory training is in date Awareness of and compliance with all relevant practice policies/guidelines, e.g. prescribing, confidentiality, data protection, health and safety Commitment to life-long learning and audit to ensure evidence-based best practice Excellent inter-personal and communication skills in a confidential manner Desirable Experience of working in primary care and/or GP practice.
