Job description
The purpose of this role is to support Cornwall Council’s Rough Sleeping team reduce the number of people rough sleeping across Cornwall.
This role will operate across Cornwall and the post holder will be expected to work in partnership with other agencies to ensure that people who are rough sleeping; homeless or threatened with homelessness are supported appropriately.
The key aims of the role are to:
- Help clients access accommodation, including bespoke solutions, to prevent them from rough sleeping
- Facilitate access to additional interventions where required and make referrals to other agencies who offer specialist support, including the Next Steps Mental Health team and the Out of Hospital Service
- Support Recovery by facilitating move on when appropriate in partnership with accommodation providers.
A vital part of this role is to support clients to access accommodation, including bespoke solutions, to prevent them from rough sleeping. This may include where appropriate facilitating Multi-Disciplinary team meetings to ensure where possible all appropriate support is being provided, and all accommodation options are explored.
