Job description
This role is full time for 3 months initially, to start ASAP and paying up to £360 per day Umbrella inside IR35. The role is not remote but client requires candidates once a week in office (visiting patient's the additional 4, can complete write ups at home).
Job Purpose:
- The post holder will act as an expert Learning Disability Nurse providing specialist clinical and care management advice and support for adults with a learning disability diagnosis who have complex needs living in, or the commissioning responsibility of, North Central London.
- The post holder will be actively involved in developing care plans and advising on appropriate packages of care in the community or supporting with stepping down from an inpatient facility. They will ensure high quality care is delivered that is effective, safe and clients and their advocates experience positively.
Duties and Accountabilities:
- Effectively manage a caseload of clients with a learning disability and complex care needs
- Provide specialist nursing advice and support to clients with a learning disability and complex care needs, their families, or other providers, clinicians or professionals involved in the care, using evidence based practice.
- Assess, identify and prioritise the physical, cognitive, emotional and behavioural needs of our clients.
- Make clinical decisions to support clients with complex and often rapidly changing specialist learning disability care needs
- Plan, implement and evaluate care plans for clients and their carers using evidence based and person-centred principles
Qualification and Knowledge Required:
- 1st level Registered Nurse with current registration with the NMC.
- Educated to degree Level
- RNLD 5 years post senior clinical experience.
- Evidence of CPD relevant to role
- Full understanding of Section status relating to mental health patients (S2/S3/S37/41/S117) and the impact on service delivery and funding streams
- Detailed knowledge of the Continuing Healthcare Framework (2022) and its application
- Awareness of NHS funded care and how its application is in practice
