Job description

An exciting opportunity has arisen for an experienced Nursing Support Worker or Therapy Assistant to progress who hasan enthusiastic interest in Adult Respiratory Medicine.

We are a seven-day service consisting of a specialist team who work primarily to meet the needs of patients with long-term respiratory conditions and also the Acute Respiratory Infection Virtual Ward.

The role will include conducting face to face assessment and reviews of people within their own homes, and a clinical environment for oxygen assessment and home safety reviews.

There is opportunity for full time hours and x2 posts available as this is a role we are expanding.

  • To support Integrated Respiratory Team staff in providing an innovative and high quality service in an environment that enhances client care.
  • To support Integrated Respiratory Team staff during the Pulmonary Rehabilitation sessions at a variety of venues.
  • To lead on carrying out home oxygen safety checks with support as needed from qualified staff.
  • To participate in the oxygen assessment clinics and walking tests with support from qualified staff.
  • To undertake a risk assessment of all venues to ensure a safe environment for patients at each venue.
  • To liaise regularly with the Integrated Respiratory Team to assist in the co-ordination and development of the care of Respiratory patients in our care.

Mid Cheshire Hospitals NHS Foundation Trust (MCHFT) provides good quality, safe and effective healthcare to the people of Cheshire and beyond.   • Assist clinicians in the management and treatment of clients under the guidance of the clinician.
  • To independently carry out specified assessment, treatment activities and programmes as planned and discussed with clinicians and using own initiative and experience to modify according to the patient’s condition.
  • Holding own caseload as delegated by clinicians, reporting back to the clinician at agreed timescales.
  • To work with an individual or with a group in whichever venue is appropriate to the client’s need, as agreed with the clinician.
  • To be responsible for the preparation of the working environment, including equipment and materials, e.g. oxygen assessment clinics. Ensuring equipment is maintained in safe working order, faults reported, and stock reordered as necessary.
  • Contribute to the care of all clients, ensuring their safety and dignity at all times.
  • To ensure accurate, up to date client case notes and records are always kept and ensure their security and confidentiality. Adhering to local documentation guidelines.
  • To liaise as appropriate with members of the multi-disciplinary and multi Agency team.
  • To liaise as appropriate with carers, and other health, social or voluntary agencies.
