We are recognized by Clinical Commissioning Groups as a leading service provider and pioneers of non-surgical weight loss. We have been delivering services across the South of England since 2008. Our services are multi-disciplinary and include psychological therapy, dietetics, and health training with the medical management of severe and complex morbid obesity for patients with a BMI of 35+. We currently have a vacancy for an enthusiastic and motivated Care Co-Ordinator to join the multi-disciplinary team to support our service users working towards bariatric surgery working closely with our Acute team. We are looking for applicants that are understanding, enthusiastic and passionate about patient care.

Main duties and responsibilities

A Care Co-Ordinator serves as the link between the patients’, hospital providers, and the TBC Healthcare service. If a patient has any questions, concerns or comments regarding their care, the Care Co-Ordinator is the first point of contact. Their primary responsibility is the patients’ satisfaction with the services received.

The Care Co-Ordinator will behighly organised,a good communicator and able to demonstrate an ability to listen with empathy. They should be a problem solver with a “can-do” attitude, a team player and should be able to communicate with professionals on all levels.

General Duties

· Entering patient data on to the in-house system (full training will be provided)

· Booking Assessments

· Liaising with health professionals, hospitals and GPs

· Overseeing patients’psychological pathways from referral to discharge

General Responsibilities

  • To be responsible for the patient’s experience for the length of time they are in the service.
  • To build relationships with and be the first point of contact to patients and to maintain excellent communication with hospital providers and GPs
  • To work with the Service Manager and Acute Case Co-ordinator and rest of the patient care team to resolve any problems that may arise.

Employment Type: PAYE

Hours: 37.5 HRS

Based at: REMOTE

Job Types: Full-time, Permanent

Salary: £20,000.00 per year

  • 8 hour shift
  • Monday to Friday
