Job description
An exciting opportunity has arisen for an enthusiastic band 3 Rehabilitation Support Worker to join our One Carehome Team based out of Edgware Community Hospital in Barnet, but mainly seeing patients in nursing and residential care homes throughout Barnet. You will be managing a varied caseload and providing patient centred care in the community to a predominantly elderly client group.
Applicants should have a relevant professional qualification and clinical experience working with older people in a rehabilitation or enablement capacity.
To undertake a rehabilitation support worker (RSW) role working as part of a community based therapy team to support elderly patients living in care homes. The Rehabilitation Support Worker will implement as appropriate the care plan developed by therapists.
The Rehabilitation Support Worker’s role will focus on improving independence and will include promotion of daily living skills, and functional independence. They will also have to deliver and fit adaptive equipment aids as prescribed by the OT.

- The One Care Home team work closely with residents in nursing and residential homes to provide rehabilitation and support services to promote independence, a return to baseline functioning for those who have recently declined, and to improve quality of life.
- The service operates Monday to Friday.
