Rehabilitation Assistant, Band 3
Community Falls Team
Part Time - 22.5 hours per week
An exciting opportunity has arisen for a Band 3 Rehabilitation Assistant to join our Greenwich Falls Prevention Team. The Greenwich Falls Prevention Team comprises of Occupational therapists, Physiotherapists, a Nurse and a Therapy Assistant Practitioner. The team is linked to the Community Rehabilitation and Short term Assessment team as well as the new Greenwich Frailty Pilot. The service will be provided in clinics, in groups for those who need strength and balance classes and in patients homes across the borough of Greenwich.
Working within the Falls prevention Team, you will work holistically with the multi-disciplinary team alongside patients who have been identified as being at risk of falls/ having sustained falls, to complete timely assessment, care planning and interventions, to help minimise and reduce the risk of further falls.
As the Rehabilitation Assistant you will be required to support the therapy and nursing staff in implementing the care plans. As part of the role you will be involved with the running of the group classes across the Borough of Greenwich. This will involve group exercise as well as education sessions. You will also be required to do home visits to follow up and implement rehabilitation/exercise programmes as prescribed by qualified staff. As part of your role you will provide and review mobility equipment/aids with the support of qualified members of the team. You will act as a source to provide health education and advice to patients, carers and their families.
You will need to be a versatile, adaptable team player who has good organisational skills. You will be required to use multiple computer systems therefore a good knowledge of IT is essential. There is a strong emphasis on CPD, reflective practice and clinical supervision within the service, supported by a named AHP professional advisor. Regular in service/external training is encouraged.
