An exciting opportunity has arisen for two enthusiastic Registered Nurses to join the community arm of the Marie Curie REACT project based in Bradford. This new service aims to prevent unnecessary admission to hospital for patients with unmet palliative care needs presenting in the Emergency Department who are identified and assessed as having care needs that can be met at home with additional support.

This is a real career opportunity to both improve the patient and family experience, alongside reducing inequalities in access to specialist palliative care across Bradford.

You will work as part of a multidisciplinary community team providing high quality, integrated, well-coordinated nursing support to patients discharged from the Emergency Department whose needs can be met by rapid response short-term intervention. The patients will all be within the last year of life and have a wide range of life limiting conditions and presentations. The workload will be varied and require regular re-prioritisation and rapid assessment which offers the postholders a great learning experience in managing multiple conditions and complications effectively.

You will use evidence-based practice to assess, plan, implement and evaluate patient care supporting a team of healthcare assistants to ensure appropriate clinical care is delivered. We are passionate about continuous improvement and are looking for nurses who share this passion to work with us to ensure the people we serve, and our staff are treated with respect and as individuals. The ability to communicate compassionately and effectively is essential as is a clearly demonstrable person-centred approach.

The nursing team requires flexible working, full and part time working will be considered.
