Department To ensure patients requiring care in their own homes or a nursing home environment have an assessment completed under the guidelines laid down in the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care and to coordinate recommendation made by the Multi-disciplinary Team regarding eligibility under the Continuing Care Framework 3. Job Purpose As a member of the Continuing Care/Funded Nursing Care Team, the post holder will be responsible for coordinating and contributing in the completion of eligibility assessment for clients to receive Continuing Care or Funded Nursing Care within the community. The post holder will be responsible for ensuring all patients resident in Nursing Homes and their own homes within Royal Borough of Kingston boundaries and with a Kingston GP, are assessed against the criteria for NHS Continuing Care, before Funded Nursing Care is considered 4. Key Result Areas - To act as the nurse specialist for NHS Continuing Care/Funded Nursing Care within YHC - To co-ordinate the assessment process in determining funding from Funded www.yourhealthcare.org Nursing Care contributions and Continuing Healthcare eligibility - To co-ordinate assessments on patients within the timescales specified by the Department of Health - To co-ordinate the review of assessments and maintain the database with the assistance of the FNC team administrator - As co-ordinator of the assessment process within Nursing Homes, the Assessor will carry out the following role: Liaise with the administrator responsible to arrange the assessment Ensure patient, and next of Kin involvement in the assessment process or that reasons why they choose not to or cannot be involved are fully documented To collate the required information and evidence to ensure full assessment of needs To ensure all required documentation has been completed - In the absence of the Team Lead to process all urgent assessment/care requirements by processing the documentation and coordinating with Social Service and Commissioning - To process complaints in accordance with Your Health Care Policy and Procedures.
- To carry out retrospective needs portrayals as requested by the Team Leader., and in line with the guidance laid down by the Independent Review Panel and national guidance - To participate in meeting with patients and next of Kin / family regarding queries, clarification or discussion on either the assessment process or appeals process - To support the review of existing processes and act as a resource on The National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care to other care professionals within Your Health Care , RBK Community Care Services, South West London and St Georges Mental Health Trust and relevant staff working in nursing homes. - To be responsible for maintaining accurate records on Care Track and Care Notes, regarding assessed patients in local nursing homes - To be aware of the multiple needs of clients and carers, acquiring knowledge of disease pathways and services available to fulfil these needs and be aware of best practice. Ability to signpost clients to NHS Universal Services if needed. - Where through the assessment process the assessor identifies clinical need or poor practice within Nursing Homes they will report these immediately to their Line Manager and take the required action to ensure the safety of the patient.
