- Assesses patients, plans and implements individual care programmes and therapeutic interventions in a range of community settings including clinics and hospitals. Manages a caseload and functions as a Care Co-ordinator for service uses with complex drug/alcohol and healthcare needs. Works within an interdisciplinary team to deliver quality care with an emphasis on promoting independence, rehabilitation and harm minimisation Carries out nursing procedures May provide clinical supervision to more junior staff, students
