Job overview

This is an exciting opportunity to join a progressive and innovative Children's Diabetes team as a Children’s Diabetes Specialist Nurse.

Future service development will include transition and a special interest in this age group will be of benefit.

You will gain an insight into a specialist role working in a community setting. The postholder will carry continuing responsibility for the management of a defined caseload of Children with Diabetes in a variety of settings e.g. primary and secondary care, home and schools, work and colleges. The postholder uses clinical judgement and autonomous decision making for the assessment of care needs, the development, implementation and evaluation of programs of care and the setting of standards of care. The jobholder willprovide specialised advanced nursing skills and expertise to a specific patient group and will lead on the development of the specialist service. The postholder will ensure cost effective use of resources, and contributes to the strategic direction of the service and nursing within the Trust.

This will be achieved through the core dimensions of the role, clinical practice, leadership, and acting as the patient advocate through audit and education.

Main duties of the job

. The postholder will

Detailed job description and main responsibilities

  • Work with children and young people predominately up to the age of 19 years with type 1,

type 2 and other forms of diabetes

  • Meet the needs of patients with unusual forms of diabetes or difficult cases.
  • Will undertake specialist assessment of patients nursing needs, plan, implement and

evaluate care delivery using clinical knowledge and clinical skills.

  • Manage a patient caseload providing expert assessment, planning and evaluation.
  • Provide specialist care and advice (insulin pumps therapy, Multiple daily injections,

carbohydrate counting, variable doses and insulin adjustments, use continuous blood

glucose monitoring and interpretation of results) etc for young people and families in

hospital/home/school/work/college via phone / face to face

  • Will need to participate and liaise with other professionals regarding safeguarding and

attendance at CAF meetings and care arrangements

  • Develop and run nurse-led clinics, using nurse prescribing
  • Autonomous clinical decision making
  • Contribute towards a multi-professional approach in the management of patients within the

specialist area, especially in relation to ongoing care needs and discharge arrangements.

  • Establish and maintain effective communication with patients and carers / relatives.
  • Support patients and their families and ensure that they receive the required information to

enable them to self-manage, lead or participate in their care delivery.

  • Recognise changes in patients’ condition that require the intervention of others and refer on

as appropriate.

  • Work with the Team Leader to promote the development of nurses undertaking extended

roles as considered appropriate.
