Job description
Work with children and young people predominately up to the age of 19 years with type 1, type 2 and other forms of diabetes Meet the needs of patients with unusual forms of diabetes or difficult cases. Will undertake specialist assessment of patients nursing needs, plan, implement and evaluate care delivery using clinical knowledge and clinical skills. Manage a patient caseload providing expert assessment, planning and evaluation. Provide specialist care and advice (insulin pumps therapy, Multiple daily injections, carbohydrate counting, variable doses and insulin adjustments, use continuous blood glucose monitoring and interpretation of results) etc for young people and families in hospital/home/school/work/college via phone / face to face Will need to participate and liaise with other professionals regarding safeguarding and attendance at CAF meetings and care arrangements Develop and run nurse-led clinics, using nurse prescribing Autonomous clinical decision making Contribute towards a multi-professional approach in the management of patients within the specialist area, especially in relation to ongoing care needs and discharge arrangements.
Establish and maintain effective communication with patients and carers / relatives. Support patients and their families and ensure that they receive the required information to enable them to self-manage, lead or participate in their care delivery. Recognise changes in patients condition that require the intervention of others and refer on as appropriate. Work with the Team Leader to promote the development of nurses undertaking extended roles as considered appropriate.
