Job description
To work as part of the Clinical Team for NHS Funded Care i across the full range of CHC/FNC roles. Organise meetings with service users, relatives, carers, and multi - professionals to facilitate client centred determinations. Ensure they are informed of the processes and outcomes of any decisions. Ensure national guidelines relating to NHS Continuing Healthcare and NHS-Funded Nursing Care as per National framework for NHS Continuing Healthcare and NHS- Funded Nursing Care, Department of Health (DH) are addressed and adhered to.
Individual responsibility to ensure determined FNCs / NHS Continuing Healthcare screening, carried out by him/her is effective, efficient and equitable for service users. Manage potential volatile situations and circumstances that provoke emotive issues with regards to potential financial implications to the resident and the family members. Enlist the support of Clinical Lead where appropriate. Submit completed documentation as per agreed policy and procedures, with particular attention to detail, due to potential grievances and further implications.
The post holder will be expected to work on his/her own initiative and to undertake additional duties, assisting with small projects as part of the wider team. Please see the attached Job Description in Supporting Documents for more information
