An exciting opportunity has arisen for a dynamic, forward thinking and self-motivated individual to join our busy nursing team on Campion Ward, South Warwickshire NHS Foundation Trust. You will be part of supportive, friendly and experienced nursing team; working within the wider multi-disciplinary team to provide high standards of patient care.

Campion Ward is a Medical Rehabilitation Ward, with upwards of 20 beds, set within the Royal Leamington Spa Rehabilitation Hospital. The ward provides care for patients with a range of medical conditions and co-morbidities, creating a diverse and varied working environment. Providing a rehabilitation ethos to all care and working closely with the MDT, you will support patients in their recovery; aiming to optimise patients level of independence, quality of life and support effective discharge planning.

As a Nurse you will be caring, compassionate and committed to providing the best service possible to our patients. You will have proven organisational, time management and communication skills and be enthusiastic. You will be a clinical role model and have high clinical standards with a passion to make a difference for patients and staff alike.

This position is suitable for both experienced nurses and those about to qualify.

To deliver a high standard of nursing care to patients undergoing surgical or medical treatment.

The post holders will be responsible for the assessment of patient care needs, and implementation and evaluation of the programme of the care.

The post holders are required to work as part of a team committed to delivering and improving a high standard of nursing care within South Warwickshire NHS Foundation Trust.

Clinical

Take clinical responsibility for patients within your allocated area of work.

Continuously assess, plan, implement and evaluate treatment, using appropriate programmes/methods of care.

To ensure a high delivery of quality care in accordance with Trust policies, procedures and guidelines. Participating in activities to improve the quality, productivity and effectiveness of care in response to local and national policies and initiatives.

To practice in accordance with the NMC Code of Professional conduct and scope of professional practice and maintain own NMC registration.

In the absence of a senior colleague, and following appropriate induction and guidance, will be expected to co-ordinate the shift, and take management responsibility for admissions, transfers and discharges to the ward or department.

Liaise with the multi-disciplinary team to ensure an effective flow and communications within the area, including being available for Board and Ward Rounds as required.

Communication

Showing empathy, understanding, and professionalism, when communicating with patients, their carers and families. Identifying individual needs and therefore utilising a range of agreed communication mediums.

Communicate effectively and professionally with colleagues and all levels of staff.

Maintain confidentiality at all times, as required by legislation and Trust policy.

Professionalism

Ensure records and documentation are maintained, kept confidential, and meets professional NMC guidelines, appropriate Trust standards, and in line with the Data Protection Act.

To act in a professional manner, and as a role model, at all times, leading by example to junior members of the team.

Demonstrates a positive and enthusiastic approach to their work in order to maintain high morale and motivation within the department. Promoting a professional and positive working environment.

Maintain and develop own professional knowledge.
