Rehabilitation Nurse – Band 5 - 22.5 hours – Community Rehabilitation Team
An Exciting opportunity has arisen for an enthusiastic and skilled practitioner who wants to be part of an exciting, developing service to provide rehabilitation to adults with mental health needs (primarily psychosis) accessing our residential rehab unit and community rehabilitation services.
This 24-hour community based service, based at Hilda Johnson House offers supported living for those moving forward with their recovery journey. The successful applicant will need to work to a rota across the working week, but may also include some evenings, weekends and night shifts if required by the service. Due to the nature of the service, lone working will be required.
The post holder will be given a full induction, regular supervision and personal development reviews with opportunities to work in other areas of the rehabilitation service to gain experience and development.
This is a part-time 22.5 hours per week post that will involve working as part of a multi-disciplinary team, overseeing the care plans, rehabilitation programmes, risk assessments and goals/outcomes of our residents, providing clinical intervention and input when required, as well as input into our MDT and the rehab pathway. The post holder will also provide supervision of our STR team and have the opportunity to supervise students of all disciplines.
Experience of working in mental health would be desirable however we would welcome applicants with the right transferable skills to consider this opportunity. The team have an extended range of multi-disciplinary members including consultant and nurse practitioner, senior nurse, psychologist, Support Time and Recovery workers, Occupational Therapists and health care support workers. You will need to demonstrate enthusiasm, innovation with an appreciation and understanding of the concept of person centred recovery.

To act as care coordinator with responsibility for the planning and delivery of care to this patient group.
To take charge of the area on a regular basis.
To act as a Change Agent and actively seek to develop practice.
To act as care coordinator for a group of patients ensuring that appropriate interventions are planned and delivered according to assessed need. To promote a positive nurse patient relationship and to engage with carers / relatives as appropriate. To have responsibility for documenting the plans of care and ensuring that plans remain current and that standards of record keeping are adhered to.
To administer and manage medicines according to policy and local protocol. To assess patient response and provide advice and education to junior staff and both patient and carers in respect of prescribed medicines.
Provide support and guidance to junior or less experienced staff in the planning and delivery of nursing care for individuals for whom they are named nurse / worker. To provide support to service users within this group at times when the named nurse / worker is unavailable. E.g. periods of annual leave sickness etc. To take an active role in auditing and maintaining standards of record keeping for this group of service user.
Maintain the therapeutic milieu, taking a proactive approach, supervising and supporting others in the same. Planning clinical activity in accordance with the needs of the patient group. To take an active role in the clinical programme including facilitation of specific group activities or individual interventions. Participating in forward planning and evaluation of the clinical programme. Having an awareness of how an individual’s behaviour and presentation can impact upon the milieu and demonstrating a positive influence through one’s own conduct.
Taking prompt action on changes in condition, liaising with other members of the clinical (Multi-disciplinary) team when necessary, reviewing nursing care and communicating any amendments to the care plan.
Develop skills in respect of specific interventions that are appropriate to the clinical area, act as a lead in developing nursing practice in relation to this.
