Job Title: Community Psychiatric Nurse

Pay Rates: £18.1- £28.96

What you will be responsible for:
- Providing robust assessment of patients (by telephone or face-to-face) who have been referred to the CMHT due to presenting with mental health difficulties.
- Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their wellbeing.
- Signposting to appropriate care providers (including primary and tertiary care). Attending daily Red Board meetings.
- Raising concerns to senior staff for support and management. CompletingRiOdocumentation and planning care for patients.
- Presenting cases to Multi-Disciplinary Team for discussion around care pathway.
- Liaison with colleagues nursing, occupational therapy, medics, psychology and admin.
- The role will involve, but not be limited to you performing the above.
You’ll have the following qualification/experience:
- A minimum of 6 months paid experiences a Community Psychiatric Nurse in an UK hospital environment within the last 3years.
- Comply with NMC Professional Registration and keep up to date with current nursing practices.
- *Full UK driving licence and own vehicle.
