Job description

We have a unique opportunity for a registered nurse who has current experience in Diabetes and is a non-medical prescriber we would offer to the best applicant without non-medical prescriber to work with us on a 9 month fixed term contract to manage the developed pathway for diabetic patients who are identified at pre-assessment as needing support to get their type 1 or 2 diabetes under control before they have their elective surgery.

You will need your line manager’s permission before applying.

You will be working closely with a consultant and dietitian to support these patients. This will be your focus for the 9 months although support to ward patients may be required to support the team.

Although you will be working in a small team with the consultant and dietician on this project there is a wider team of nurse specialists consultants and registrars to support and help you.

You will be expected to attend all team meetings. – you will be part of the inpatient diabetes team to review patients post-operatively to ensure they have no problems prior to going home.

Working with the consultant you will receive the referrals and triage them, arrange for an appointment with the patient to review their disease control, using monitoring and pumps you will train the patient and their family to manage their disease prior to surgery.

You will follow the patients until safe for them to have elective surgery when they will be referred back to the anaesthetics and pre-op team.

To manage the supply of sensors to ensure appropriate usage

You will be part of the review of the pathway working with the team to provide audit data both qualitative and quantative to the divisional team and the CCG.

You will need current knowledge and experience of managing diabetes and glucose levels as the project will immediately start on successful appointment.

As part of the role and integration within the diabetes inpatient team you will be required to follow up of patients post operatively from a diabetes management perspective and make treatment adjustments

The role will involve working closely with a consultant and dietician within a multidisciplinary clinic. You will be required to support patients prior to them undergoing elective surgery to optimise their disease to support the best outcome post surgery.

You will need to have current knowledge and be working as a diabetes nurse specialist, be a non-medical prescriber and have a good knowledge of the medications and systems used in supporting patients with diabetes.

You will be required to work autonomously and run nurse led clinics where treatment initiation and changes are made to improve post-surgical outcomes.

You will need to support and encourage patients to manage and control their diabetes.

You will need to manage your caseload, accept referrals ensure the patients are booked for appointments to assess their disease and follow them through their journey.

You will need to have good communication skills to facilitate treatment changes and blood glucose management.

Must have knowledge and understanding of teaching skills required to facilitate commencing insulin and its adjustment

You will need to work independently and as part of a team.
